Application Form
If all goes well with your assessment with Levitate, you can download and fill out our application form. It’s available to download below in both PDF and Microsoft Word formats to best suit your needs. We truly appreciate all the information you provide us, as it helps us determine how we can best support your goals.
Once you’ve wrapped up your application, you can easily send it our way by posting it to the address listed at the bottom of the form or by sending it via our email. We can’t wait to hear from you!

We Will Be In Touch
A letter or email acknowledging we have received your application will be sent to you as soon as we are able to following receipt at our office. If your application does not fit the criteria we will tell you in this letter or email within 8 weeks.
Please note that our income is derived from fundraising and donations and we are likely to receive more applications than we can fund. As our funds are limited we have to prioritise and where it is deemed that the beneficiary or the beneficiary’s family are able to provide the equipment from their own resources, the application will be rejected.
